Las Animas Huerfano Counties District Health Department is currently working to complete Phases 1A, 1B.1 and 1B.2 of the COVID-19 vaccination process as outlined by the State of Colorado.
It will likely be this spring before the Pfizer or Modern vaccines are available in quantities for mass vaccination of all who want it.
The Las Animas-Huerfano Counties District Health Department has launched an online form to facilitate registering for a vaccine.
The form can be found at https://form.jotform.com/210086599112051.
It can also be found at the health department website.
“While filling out this form will inform us of the number of people in each phase, it does not guarantee a vaccine in any timeframe,” local health officials said in a press release. “When your time comes in the phases, you will be notified to schedule an appointment.”
Those seeking an appointment to receive the vaccine should call the health department’s Walsenburg office at (719) 738-2650, the Trinidad office at (719) 846-2213, or fill out the department’s online form at https://form.jotform.com/210086599112051.

In addition, Huerfano County residents can call Spanish Peaks Regional Health Center and Las Animas County residents can reach out to Mt. San Rafael Hospital, Salud Family Health Centers, or the pharmacies at Safeway and Walmart in Trinidad to make an appointment.
New Mexico residents can register to receive the vaccine by filling out this New Mexico Department of Health online form.
